Triple arthrodesis

Triple arthrodesis is a surgical procedure designed to correct flat foot deformity, a condition where the arch of the foot collapses, often leading to pain, instability, and difficulty walking. This procedure involves fusing three joints in the hindfoot: the subtalar, talonavicular, and calcaneocuboid joints. By doing so, it aims to realign the foot, restore the arch, and alleviate symptoms.

Flat foot, or pes planus, can be congenital or acquired. In adults, it often results from posterior tibial tendon dysfunction (PTTD), arthritis, or trauma. The deformity is characterised by a flattened medial longitudinal arch, valgus hindfoot, and forefoot abduction. Symptoms include pain, especially along the medial arch and ankle, and difficulty with activities like walking or standing for long periods.

Non-surgical treatments are typically the first line of management. These include orthotic devices, physical therapy, and pain management. However, when these fail, surgical intervention may be necessary. Triple arthrodesis is considered for patients with severe deformity, arthritis, or when other surgeries have failed.

The procedure involves an incision on the lateral side of the foot, allowing access to the three joints. The joints are then debrided and prepared for fusion. The foot is realigned, and the joints are fixed with screws or plates, holding them in the corrected position. Bone grafts may be used to promote fusion.

Postoperative recovery is extensive. Patients are typically non-weight-bearing for 6-8 weeks, followed by gradual weight-bearing and physical therapy. Fusion can take 3-6 months, and full recovery may take up to a year. Complications can include nonunion, infection, and nerve damage.

Despite the risks, triple arthrodesis can be highly effective. Studies show significant improvements in pain, function, and quality of life. A 2018 study in the Journal of Bone and Joint Surgery reported 90% patient satisfaction and significant pain reduction.

Triple arthrodesis is a valuable treatment for severe flat foot deformity, offering realignment, pain relief, and improved function. With careful patient selection and meticulous surgical technique, it can be a life-changing procedure.
